New Patient Referral Process
If you are a patient, physician's office, claims adjuster, urgent care, or nurse case manager, we REQUIRE the following information:
authorization from the insurance carrier specifying the physician and role (PTP, Secondary Physician, etc.), DOI and accepted body parts, patient demographics, phone number, and billing remit to address.
If you do require an immediate appointment, please specify and we will provide an appointment as soon as possible.
ADJUSTERS AND NURSE CASE MANAGERS
Our physicians are trained and experienced in treating all types of work-related injuries, as well as helping employees get back to work faster without losing valuable work hours.
We realize the importance of selecting the treating physician with appropriate skills, knowledge, and sensitivity in managing the work-related injury to the satisfaction of the injured worker and the employer. The injured worker's response to the treating physician on the initial visit is a powerful indicator of the injured worker's subsequent recovery course.
Quality care that is integrated with disability management optimizes the outcome for an injured worker. Communication among the injured worker, his or her employer, and the claims adjuster is the key to successful rehabilitation after a work injury.
